This plan offers robust coverage with reasonable
first-dollar benefits and low out-of-pocket cost.

• $30 copayments for urgent care, physician, and
naturopathic office visits
• Combined $1,000 maximum acupuncture and chiropractic care
benefit
• Annual deductibles from $500 to $10,000
• No overall lifetime limit
Elect Value Option PPO
Get peace of mind with this basic coverage, provided at a
low monthly rate.
• Annual deductibles of $2,500, $5,000, $7,500 or $10,000
• Most in-network services covered at 60% after the
deductible
• In-network prescription drug coverage at 50% after the
deductible
• No overall lifetime limit
